The error is here:
Error in evaluate_drive_within_mountlist: /dev/cciss/c0d0 does not exist.
So you loaded the hpsa driver first, and probably what you really need is cciss first.
Try disabling the hpsa driver (at boot with denymods="hpsa" or during backup with a mindi.conf option with DENY_MODS).
